Job Summary: Provides technical, organizational and interpersonal skills necessary to efficiently and effectively coordinate the scheduling and communication of all aspects of the plan of care.

Primary Responsibilities:

  1. Coordinates the implementation of all participant services to ensure that quality service is provided to meet participant needs.
  2. Ensures communication and coordination with home care staff.
  3. Ensures clinical care is provided according to orders and the care plan.
  4. Coordinates with Site Manager to oversee the provision of ancillary and specialty services.
  5. Coordinates with ancillary providers including dentist, podiatrist, pharmacist, audiologist and ophthalmologist.
  6. Coordinates with ancillary and specialty services to arrange appointments and share pertinent information.
  7. Coordinates nursing care for PACE participants in the nursing home, foster care and other facilities according to program standards. 
  8. May participate in inter-agency meetings as needed.
  9. Participates with the Quality Improvement Program to determine opportunities for improvement and support continued performance improvement.
  10. Participates in quality improvement studies according to the quality plan.
  11. Recommends studies for the annual quality improvement plan.
  12. Ensures completion of documentation and delivery of clinical services reviewing medical record.
  13. Knows and adheres to the philosophy of Northland PACE.
  14. Keeps confidentiality of participant records, reports and discussions.
  15. Participates in Northland PACE Committees as requested by Site Manager.
  16. Advises the Director of Nursing in ways and means to establish better accountability of Northland PACE services to participants and referral sources.
  17. Attends and participates in scheduled staff meeting and client care meetings as requested.
  18. Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload in order to fulfill responsibilities.
  19. Informs Director of Nursing of “unusual occurrences.”
  20. Identifies nursing service delivery problems and uses good judgment in their solution.
  21. Is professional in appearance and manner in the clinical area; recognizes own limits and seeks help and guidance from the Site Manager as appropriate; serves as a role model for staff members.
  22. Maintains applicable licensure and certification and pursues professional growth through continuing education process.

Employment Specifications:

 

Education/Experience:

Bachelor’s Degree required.  Healthcare experience preferred.  Prior experience with workflow processes required. 

Licensure, registry or certification required:

None.

Special Training:

 

Working knowledge of the interdisciplinary model of care management.  Experience working with cognitively impaired seniors is important.  Must be able to relate well to seniors and their families to deal with sensitive issues and facilitate problem solving.  Must be flexible and able to change easily.

Physical Demands:

Must be able to see, hear, and verbally communicate.  Must be able to move about freely and possess manual dexterity and visual acuity. Moderate amount of walking.  Must be able to move about freely, lift and turn participants.

Working Conditions:

Primarily inside, well-lit, climate controlled environment.  Occasional travel to homes, other sites or contracted facilities.  Potential exposure to infectious diseases and environmental hazards.
