Job Summary: Performs various participant care activities.


Primary Responsibilities: The Home Care CNA’s duties shall include, but not be limited to:



  1. Provides all appropriate cares to participants in Participants’ homes as scheduled by the CNA Scheduler; including but not limited to toileting, personal care, foley care, colostomy care, medication reminders, bathing and transfers or positioning.
  2. Provides all appropriate cares to participants in the Day Center as needed and as scheduled by the CNA Scheduler.
  3. Assists with transporting and escorting of participants to outside medical appointments as needed.
  4. Assists with transporting of participants to and from the day center as needed.
  5. Assists participants to transfer and ambulate as instructed by Nursing or Physical Therapy.
  6. Takes and records vital signs; including weights and alerts Clinic Nursing or Home Care Coordinator of any vitals that are out of range or any changes in weight.
  7. Provides assistance with 1:1 or list shopping for Participants.
  8. Provides light housekeeping and laundry services for Participants in their homes.
  9. Maintains a clean, neat environment to enhance participant safety.
  10. Ensures that supplies and equipment necessary for patient care are stored in an organized, efficient and safe manner.
  11. Is an advocate for participants enrolled in the Northland PACE Program.
  12. Consults with Clinic Nursing, Home Care Nursing or Director of Nursing regarding problems that require increased nursing input.
  13. Follows the individualized care plans for Participants’ as written by the Home Care Coordinator.
  14. Keeps abreast of current Certified Nursing Assistant knowledge, especially in the field of geriatrics by attending professional conferences and in-services.
  15. Participates in interdisciplinary team meetings and staff meetings as needed.
  16. Understands and follows Northland PACE Program policies and procedures.
  17. Demonstrates service excellence to all participants in job performance.
  18. Other duties as assigned

Employment Specifications:


High school diploma or equivalent.




At least one year’s experience with a frail or elderly population

Number and type of employees supervised:




Licensure, registry or certification required:


Certified Nursing Assistant in ND.

Current, valid North Dakota driver’s license.


Special Training:


Must be able to work effectively in a team environment. Must be able to communicate effectively. Must possess strong interpersonal and organizational skills. Ability to adapt to change in a positive/ professional manner. Must meet standardized competencies established for position. Must be medically cleared for communicable diseases. Must have all immunizations up to date before engaging in direct participant contact.


Physical Demands:

Must be able to see, hear, and verbally communicate. Must be able to move about freely and possess manual dexterity and visual acuity. Moderate amount of walking. Must be able to move about freely, lift and turn participants.


Working Conditions:

Primarily inside, well-lit, climate controlled environment; however, will require travel in various weather elements (rain, snow, cold, warm temperatures etc). Frequent travel for home visits. Must have reliable transportation. Potential exposure to infectious diseases and environmental hazards.
