Eligibility

Northland PACE provides comprehensive care to older adults in the community who qualify for Medicare, Medicaid or those who desire to privately pay for our services. All interested applicants are first screened by the Northland PACE Intake staff to verify basic eligibility, and the Northland PACE Team completes assessments to determine a prospective participant’s safety in the community.

Prospective participants must meet the following criteria to be eligible to participate in the PACE Program:

• At least 55 years old

• Living within one of the Northland PACE approved service areas:

• Qualify for nursing home level of care

• Able to live safely live in the community with PACE support at time of enrollment
