Overview

Primary Responsibilities:

  1. Performs initial nutrition screening upon assessment, developing care plans and monitoring participant progress.  Reassessment is done every six (6) months thereafter.  Interacts with IDT members, as appropriate, to accomplish these tasks at day center, nursing home or participant’s homes.

  2. Monitor monthly weights, taking into account medical diagnosis, drug nutrient interactions, changes in chewing, swallowing, mood changes, hydration status, intake, skin breakdown.

  3. Provides nutrition counseling for Northland PACE participants, using appropriate materials.  Discusses diet principles and other information as appropriate about shopping, food preparation, etc.

  4. Participates in educational activities, developing or presenting monthly group classes; developing or selecting/improving educational materials; or presenting in-services to own department or other departments.

  5. Participates in patient care conferences, multidisciplinary team actives, committees, etc. as appropriate.  Consults with MD/RNP and staff, making appropriate recommendations; treats all people with respect, maintains confidentiality of Northland PACE participants, and strives towards excellence.

  6. Monitor portion control; appropriate serving of food.

  7. Works on projects for the department, small or large, as appropriate; participates in department quality improvement activities.

Employment Specifications:

Education/Experience:  Minimum of Bachelor of Science in nutrition, and Registered Dietitian.  Minimum of 3-5 years’ experience as a registered dietician, including clinical and administrative responsibilities preferably in geriatrics.

Licensure, registry or certification required:  Registered Dietitian with the Commission on Dietetic Registration of the American Dietetic Association.

  2. Determines participant and family needs related to social support, financial support, counseling and housing.
  3. Confers with participant and family to identify participant goals and expectations.
  4. Coordinates with the interdisciplinary team to develop a comprehensive care plan for each participant.
  5. Provides individual and group counseling to participants and their families as needed or prescribed in the care plan.
  6. Coordinates the completion of participant’s health care wishes and advance directives in cooperation with their primary care physician, the participant and their family.
  7. Provides discharge planning in the event of disenrollment.

  10. May assist in facilitating the Participant Council to create a vehicle for dialogue between participants and the Care Team and empower participant responsibility.

Job Summary:  Under the general direction of the Northland PACE Program Board of Directors and the Executive Director, responsible for the development, implementation and ongoing monitoring of the Quality Improvement (QI) Program.

Primary Responsibilities: The duties of the Quality Management Coordinator shall include, but are not limited to:  

  1. Gain Northland PACE Board approval of the annual QI plan.
  2. Develops and implements annual Quality Improvement (QI) plan, inclusive of associated audits, in conjunction the Quality Improvement Steering Committee (QISC), which is inclusive of the President, Executive Director, Medical Director and Compliance Manager.
  3. Provide for monthly review (CMS regulation) of indicators contained within the QI plan which may include correspondence, memos, reports, maintenance of files and/or records pertaining to QI plan and the Quality Improvement (QI) Teams/QISC.
  4. Collaborates with Executive Director and Medical Director to plan, organize, direct, and lead personnel and work processes of the QI plan and related initiatives.
  5. Engage leadership, staff, and providers in QI methodology such as PDSA to align critical work processes, performance and outcome goals, and regulatory requirements.
  6. Maintains all processes associated to the maintenance of the Northland PACE Policy and Procedure Manual, ensuring the proper review and approval of all policies through the QISC.
  7. Compliance to policy and procedure as spelled out within the Northland PACE Policy and Procedures through documented surveillance, data collection and analysis; responding to identified issues, reporting and making recommendations to the QISC and Executive Director.
  8. Conducts oversight of the trending analysis of participant and employee satisfaction and feedback surveys. Reports participant and employee satisfaction trending to Executive Director for QI opportunities.
  9. Has responsibility for planning, preparation, and reporting in relation to the Center for Medicare and Medicaid Services (CMS) PACE Audit Process and Data Request to ensure readiness for CMS Audits and ensures that Corrective Action Plans (CAPS) are implemented, followed, audited and recorded in a timely manner.
  10. Oversight of and accountable for all required reporting to CMS via HPMS, SAA, board of directors, participant advisory committee, staff, participants, and ensures accuracy of data being reported.
  11. Review of and track the completion of Level I and II Reporting.
  12. Accountable to ensure all required DataPACE3 indicators and ensures accuracy of data being reported.
  13. Assist in the development of and responsible to sustaining risk management programming. Utilizing data collected on adverse events, near misses, and unsafe conditions, and at risk behaviors to report and identify opportunities for staff education, leadership involvement for systems improvement, to reduce probability of future recurrence, and performance of root cause analysis.
  14. Completes all other duties as assigned; The position may be modified as structure and implementation may require the addition of needed duties to be more effective with quality in the near future.

Education:  Baccalaureate Degree in health-related field from a recognized college or university.

Experience:  At least 5 years’ experience in quality management.

Licensure, registry or certification required:  Certified Professional in Healthcare Quality (CPHQ) preferred, not required.

  6. Maintains vehicle in neat and clean condition anticipating needed mechanical service so appointments can be arranges that are convenient to program operations.  Keeps vehicle maintenance records current; coordinate and schedule routine episodic vehicle maintenance.
  7. Immediately reports to Site Manager any vehicle operating problems, malfunctions or incidents out of the ordinary routine.
  8. Maintains accurate daily records of number of trips, individuals transported, miles and gallons of gas.  Complies all necessary information to meet reporting requirements.

  2. Manages the service delivery to participants.
    1. Ensures the efficient operations of activities, nutrition, personal care, clinical services, rehabilitation, and other services of the center within the standards and regulations of the program.

  5. Manages the facility support for a safe and comfortable environment.
    1. Ensures the appropriate maintenance of equipment and furnishings in the center.
    2. Ensures that the appropriate equipment and supplies are available, including recommendations for capital equipment and authorization and oversite of purchasing for small equipment and supplies.
    3. Ensure that infection control, fire safety and related safety procedures are followed.
    4. Ensures durable medical equipment is maintained in safe, usable condition.
  6. Coordinates with contract services to ensure understanding of the PACE model and cooperation with the care management process.
    1. Ensures the coordination with home health, long term care and medical services according to participant care plans.
    2. Assists in the development of agreements and contracts with provider agencies to ensure comprehensive service availability.
  7. Other responsibilities/requirements.
    1. Serves as an active member of the Northland PACE Program management team, and as such, serves as a liaison between Northland PACE Program administration and direct staff.

Employment Specifications:

Education/Experience: Preferred B.S. in Nursing, Social Work, Public Health or related healthcare/gerontology field or 1-5 years of relevant experience.  Experience may substitute for education requirement.

Licensure, registry or certification required:  Must maintain current licensure/certification within scope of practice.

Physical Demands:  Must be able to see, hear, and verbally communicate.  Must be able to move about freely and possess manual dexterity and visual acuity.

Job Summary: The Medical Administrative Assistant provides support to the PACE Site Director, managers, supervisors, other employees and PACE Participants by handling a variety of tasks in order to ensure that all interactions between the organization and other are positive and productive.
 
Primary Responsibilities The Medical Administrative Assistant performs a wide range of administrative and office duties to ensure the smooth and efficient daily operations of the PACE Program. 
  1. Answer, screen and transfer inbound calls
  2. Receive, monitor and direct participants and visitors to the PACE site
  3. General clerical duties including copying, faxing, emailing, filing, creating correspondence, etc.
  4. Maintain office, medical and housekeeping supply inventory
  5. Reconcile expense reports and credit card accounts
  6. Sort and distribute mail
  7. Coordinate repairs for office equipment
  8. Schedules Participant appointments
  9. Maintains Participant Medical Records:
    1. Scans documents in a timely manner into the electronic medical record, using established processes to ensure information is available for participant care.
    2. Performs quality control to ensure the integrity, completeness and legibility of scanned patient health records by validation of the paper records to the scanned images.
    3. Purges and archives medical records on an as needed basis.
    4. Ensures access to participant medical records, for continuity of care, while maintaining security and confidentiality of the medical record.
    5. Prepares for PACE provider clinic appointments by printing summary information for the provider prior to the appointment.
    6. Facilitates submission of PACE clinic and nursing home visit reports to the coding vendor and retrieval of completed coding summaries from the coding vendor for all three sites.
    7. Reviews and enters ICD-10 diagnoses codes, from the coding vendor, in to the electronic medical record.
    8. Works with providers to ensure risk adjustment data is complete, accurate, and timely.
    9. Obtains past medical records for potential enrollees as part of the intake process, using access to the NDHIN, external EMR systems, and release of information requests for all applicable facilities.
    10. Accesses outside facility electronic records for reports of emergency care and hospitalizations for scanning to the PACE electronic medical record system, obtaining the reports via record request to facilities which do not provide PACE access to the electronic record.
    11. Obtains office visit documentation for all outside medical appointments, using NDHIN, external EMR systems, and release of information requests.
    12. Monitors the completion of assessment documentation for all care conferences, sending emails communications, per policy.
    13. Performs monthly chart audits and CAPS assessment audits to support QAPI activities. 
    14. Monitors, regularly, for the accurate entry and delivery of emergency room, ambulance, and hospitalization services into PCO, as well as for completion of associated change forms.
    15. Provides support for the quality audits by retrieving data from PCO on a monthly basis for the on-call log audit.
    16. Performs the Release of Information function for PACE in accordance with established policies and procedures, adhering to confidentiality policies and HIPAA regulations.
    17. Maintains the Health Outcomes Survey-Modified contact spreadsheet, for Medicare eligible PACE Participants, for provision to the Centers for Medicaid and Medicare Services survey vendor.

Job Summary: Promotes and maintains the health of enrolled participants through nursing assessment, treatment, teaching and counseling.  Responsible for completing enrollment and periodic assessments of health needs to contribute to the individual’s plan of care.   6. Collaborates with the Home Care Nurse about teaching needs of participants and families
  7. Implements, directs, evaluates and provides oversight of nursing care and care provided by certified nursing assistants.
  8. Collaborates with provider to facilitate PACE appointments, participant needs and plans/follow-up for specialty appointments and medical orders
  9. Assists with preparation for interdisciplinary team meetings and ensuring that all updates for participants and needed appointments are added to the schedule
  10. Communicates regularly with pharmacy regarding prescription changes and ensuring that participant MAR is up to date
  11. Provides care to participants in a positive, nurturing, professional manner.
  12. Provides health teaching to participants with complex nursing needs
  13. Triages and ensures assessments are completed on participants reporting any sudden illness or onset of illness at home.
  14. Is flexible and able to adjust schedule to meet complex needs of participants.
  15. Consults with other staff members regarding any concerns about appointments, complaints or other anticipated/discovered needs identified during assessments.
  16. Works with social worker regarding family and caregiver in caring for homebound participants.
