Job Summary:  Under the general direction of the Northland PACE Program Board of Directors and the Executive Director, responsible for the development, implementation and ongoing monitoring of the Quality Improvement (QI) Program.

Primary Responsibilities: The duties of the Quality Management Coordinator shall include, but are not limited to:  

  1. Gain Northland PACE Board approval of the annual QI plan.
  2. Develops and implements annual Quality Improvement (QI) plan, inclusive of associated audits, in conjunction the Quality Improvement Steering Committee (QISC), which is inclusive of the President, Executive Director, Medical Director and Compliance Manager.
  3. Provide for monthly review (CMS regulation) of indicators contained within the QI plan which may include correspondence, memos, reports, maintenance of files and/or records pertaining to QI plan and the Quality Improvement (QI) Teams/QISC.
  4. Collaborates with Executive Director and Medical Director to plan, organize, direct, and lead personnel and work processes of the QI plan and related initiatives.
  5. Engage leadership, staff, and providers in QI methodology such as PDSA to align critical work processes, performance and outcome goals, and regulatory requirements.
  6. Maintains all processes associated to the maintenance of the Northland PACE Policy and Procedure Manual, ensuring the proper review and approval of all policies through the QISC.
  7. Compliance to policy and procedure as spelled out within the Northland PACE Policy and Procedures through documented surveillance, data collection and analysis; responding to identified issues, reporting and making recommendations to the QISC and Executive Director.
  8. Conducts oversight of the trending analysis of participant and employee satisfaction and feedback surveys. Reports participant and employee satisfaction trending to Executive Director for QI opportunities.
  9. Has responsibility for planning, preparation, and reporting in relation to the Center for Medicare and Medicaid Services (CMS) PACE Audit Process and Data Request to ensure readiness for CMS Audits and ensures that Corrective Action Plans (CAPS) are implemented, followed, audited and recorded in a timely manner.
  10. Oversight of and accountable for all required reporting to CMS via HPMS, SAA, board of directors, participant advisory committee, staff, participants, and ensures accuracy of data being reported.
  11. Review of and track the completion of Level I and II Reporting.
  12. Accountable to ensure all required DataPACE3 indicators and ensures accuracy of data being reported.
  13. Assist in the development of and responsible to sustaining risk management programming. Utilizing data collected on adverse events, near misses, and unsafe conditions, and at risk behaviors to report and identify opportunities for staff education, leadership involvement for systems improvement, to reduce probability of future recurrence, and performance of root cause analysis.
  14. Completes all other duties as assigned; The position may be modified as structure and implementation may require the addition of needed duties to be more effective with quality in the near future.

Education:  Baccalaureate Degree in health-related field from a recognized college or university.

Experience:  At least 5 years’ experience in quality management.

Licensure, registry or certification required:  Certified Professional in Healthcare Quality (CPHQ) preferred, not required.

Special Training:  Must be able to work effectively in a team environment.  Must be able to communicate effectively.  Must possess strong interpersonal, organizational skills and process management skills. Ability to adapt to change in a positive/professional manner.

Physical Demands:  Must be able to see, hear, and verbally communicate.  Must be able to move about freely and possess manual dexterity and visual acuity.  Moderate travel, local, state and national. Sedentary for long periods of time.

Working Conditions:  Primarily inside, well-lit, climate controlled environments, including all PACE sites, and NHA businesses.
