Job Summary:  Responsible for providing safe transportation to participants in a timely, competent manner and communicating important information relevant to the overall care plan to the interdisciplinary team.

Primary Responsibilities: Under limited supervision, and in accordance with all applicable federal, state and local laws/regulations:

  1. Transports in a safe manner participants to and from the Day Center on each day they are scheduled to attend, in compliance with ND Department of Motor Vehicle licensing regulations and local statues.
  2. Provides physical assistance to each participant when needed and they board and disembarks from the program’s vehicles.
  3. Ensures that emergency medical information is available for those participants being transported.
  4. Maintains the first aid kit in the vehicle, replacing supplies as they are used or become outdated.
  5. Ensures that all passengers wear safety belts and wheelchairs are locked into position
  6. Maintains vehicle in neat and clean condition anticipating needed mechanical service so appointments can be arranges that are convenient to program operations.  Keeps vehicle maintenance records current; coordinate and schedule routine episodic vehicle maintenance.
  7. Immediately reports to Site Manager any vehicle operating problems, malfunctions or incidents out of the ordinary routine.
  8. Maintains accurate daily records of number of trips, individuals transported, miles and gallons of gas.  Complies all necessary information to meet reporting requirements.
  9. Transports participants to and from physician or other medical appointments, as requested by the Interdisciplinary Team via the care plan.
  10. Assists with program activities by transporting participants to an event, as coordinated by the activities director.
  11. Provides courier services for supplies, interoffice mail, etc.
  12. Knows and adheres to the philosophy of Northland PACE.
  13. Keeps confidentiality of participant records, reports and discussions.
  14. Participates in Northland PACE Committees as requested by Site Manager.
  15. Attends and participates in scheduled staff meeting and client care meetings as requested.
  16. Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload in order to fulfill responsibilities.
  17. Informs Site Manager of “unusual occurrences.”
  18. Identifies service delivery problems and uses good judgment in their solution.
  19. Is professional in appearance and manner in the clinical area; recognizes own limits and seeks help and guidance from the Site Manager as appropriate.
  20. May serve as a role model for new trainees.

Employment Specifications:

 

Education/Experience:

High school diploma or equivalent.  Some experience with the elderly is desirable.

 

Licensure, registry or certification required:

Current, valid North Dakota driver’s license.  Clean driving record.

 

Special Training/Skills:

 

Must be able to read, write and speak English and be understood.  Must be able to handle information in a confidential manner at all times.  Must be able to deal professionally and diplomatically with people both in person and on the telephone.  Must have good interpersonal skills.  Ability to perform wheelchair and ambulatory transfers between home and van, and Day Center and van.

 

Physical Demands:

Must be able to see, hear, and verbally communicate.  Must be able to move about freely and possess manual dexterity and visual acuity. Moderate amount of walking.  Must be able to move about freely, lift and assist participants with transferring from vehicle to facility.

Working Conditions:

Frequently outside of facility in various weather conditions (rain, snow, cold, warm temperatures, etc.).  Potential exposure to infectious diseases and environmental hazards.
