Social Work Designee

Job Summary: The Social Work Designee supports the Northland PACE Socials Work department by assisting participants with their basic needs, coordinate social services and act as a representative for participants who are unable to advocate for themselves. 

Primary Responsibilities: Under the supervision of the Social Work Director, the Social Work Designee:

  1. Provides administrative support for the Social Work department.
  2. Assists participants in identifying and obtaining available benefits and social and community services.
  3. Assists with daily tasks in the Social Work department.
  4. Schedules, obtains, and/or coordinates services for participants.
  5. Assists with arranging meetings with relevant agencies involved in care of the participant.
  6. Serves as an advocate to help participants as needed.
  7. Transports and accompanies participants to appointments.
  8. Assists participants with preparation of forms or applications in the effort of obtaining outside services.
  9. Observes participant activities and document information as required.
  10. Understands and adheres to the philosophy and goals of Northland PACE.
  11. Maintains a quiet and safe environment for participants, visitors and staff.
  12. Upholds confidentiality of participant records, reports and discussions.
  13. Participates in Northland PACE Committees as requested by the Social Work Director.
  14. Maintains flexibility in schedule and responds to unexpected emergencies and change in workload in order to fulfill responsibilities.
  15. Is professional in appearance and manner from the Social Work Director as appropriate.
  16. Serve as a role model for new trainees.

Employment Specifications:



High school diploma or GED. Preferably have experience in a customer service role. Prefer knowledge or experience in community social services and resources.

Special Training/Skills:


Ability to demonstrate effective interpersonal skills, having integrity, being tactful, courteous and sensitive with internal and external contacts.  Must be able to adapt to rapid change and must be able to intervene promptly in crisis/emergency situations.  Ability to communicate verbally and in writing; able to follow verbal and written instructions and problem solve effectively with participants, family members and staff. Ability to document accurately and timely. Must maintain confidentiality.


Physical Demands:

Must be able to see, hear, and verbally communicate.  Must be able to move about freely and possess manual dexterity and visual acuity. Moderate amount of walking.  Must be able to move about freely, lift and turn participants.


Working Conditions:

Primarily inside, well-lit, climate controlled environment; however, will require travel in various weather elements (rain, snow, cold, warm temperatures etc.). Frequent travel for home visits. Must have reliable transportation.  Potential exposure to infectious diseases and environmental hazards.
