Job Summary: Responsible for all nursing services provided to participants in the home as assigned by the Director of Nursing or Home Care Coordinators.

 

Primary Responsibilities: The Home Care Nurse’s duties shall include, but not be limited to:

 

  1. Provides nursing care for participants enrolled in the Northland PACE Program as directed by Director of Nursing or Home Care Coordinators.
  2. Assesses participants’ needs and reports to Home Care Coordinator or Director of Nursing and carries out appropriate nursing care as needed.
  3. Performs evaluation of episodic complaints. Refers to other providers as appropriate.
  4. Provides nursing care in the home setting as care plan and Nursing team deems necessary based on assessment/reassessment of participant needs.
  5. Participates in preventative nursing care procedures as appropriate for the participants’ care and safety.
  6. Assists in reviewing charts to coordinate participant care and sees that the plan of care is carried out.
  7. Is flexible and able to adjust schedule to meet complex needs of participants.
  8. Charts all provided interventions in the electronic medical record.
  9. Provides coverage for routine evening and weekend cares as scheduled by Director of Nursing.
  10. Is an advocate for participants enrolled in the Northland PACE program.
  11. Consults with Director of Nursing and Personal Care Attendants regarding problems that require increased nursing input.
  12. Provides health teaching to participants with complex nursing needs.
  13. Keeps abreast of current nursing knowledge, especially in the field of geriatrics, by attending professional conferences and in-services.
  14. Participates in staff meetings.
  15. Understands and follows Northland PACE Program policies and procedures.
  16. Demonstrates service excellence to all participants in job performance.
  17. Maintains accurate records of visits for self to be used for data collection.
  18. Other duties as assigned.

 

Employment Specifications:

Education:

Associate degree or Bachelor’s degree in nursing from an accredited college or university.

 

Experience:

 

At least two years as staff nurse; home and/or community health preferred.

 

One year’s experience with a frail or elderly population

None

 

 

Licensure, registry or certification required:

Current North Dakota nursing license.

Current, valid North Dakota driver’s license.

 

Special Training:

 

Must be able to work effectively with an interdisciplinary team. Must be able to communicate effectively. Must possess strong interpersonal and organizational skills. Ability to adapt to change in a positive/professional manner. Must possess the ability to effectively handle difficult and unusual interpersonal situations. Must meet standardized competencies for standards of medical care and established for position. Must be medically cleared for communicable diseases. Must have all immunizations up to date before engaging in direct participant contact.

 

Physical Demands:

Must be able to see, hear, and verbally communicate. Must be able to move about freely and possess manual dexterity and visual acuity.

 

Working Conditions:

Primarily inside, well-lit, climate controlled environment; however, will require travel in various weather elements (rain, snow, cold, warm temperatures etc). Frequent travel for home visits. Must have reliable transportation. Potential exposure to infectious diseases and environmental hazards.
